Here's What You Need:
- Name, address, and social security number
- Employer(s) name(s), and work number(s) for the past two years
- Proof of income including bonuses, commissions, and overtime income for the past two years
- If you are self-employed, the last two years' tax returns: Sole Proprietorship (Schedule C), Partnership (From 1065), or Corporation (Form 1120 or 1120s).
- The last two years' personal tax returns (including K-1s)
- Documentation for credit history problems (if applicable): late payments, bankruptcy, defaults, judgments, and/or liens.
